It is not uncommon for me to get requests from patients for the desire to turn a round face into a more beautiful look. One of the reasons for a round face is overweight patients. Full neck, round cheeks, and a lot of subcutaneous fat around the face create a general cleverness that creates a fat, round face. These patients know full well that they are overweight and some liposuction of the neck and buccal lipectomies can make a difference. But weight loss might make the most difference in this case. And this procedure cannot be performed until some weight loss has been achieved.
But overweight patients are not the ones that make the patient 'round face'. Usually these younger patients do not have excess weight and are often closer to being taller and heavier. They may have mild fatigue on the cheeks and neck but often the sharpness of their bones (cheeks, chin, or jaw angle) may be a bit lacking. The approach to these patients must address both hard and soft tissue issues in order to effectively improve the angularity and definition of the face.
In addition to the sensitivity of the cheeks and neck, the next most important consideration in improving facial shape should be on the chin. Usually it may be short in horizontal dimensions but the width of the chin is also important, especially in male patients. Chin implants today can provide more projections and widths in various sizes. Placed under a small spine under the chin bone, chin implants can lend more definition and awareness to a less-than-dramatic face.
The upper facial fatigue, more specifically the midfacial sensitivity, can be achieved through the cheek implants.
